Records are $10 per copy in person and $15 per copy via the mail. Records that are over 100 years old (1923 or older) are considered genealogical requests and the search fee ($10 per record in person/$15 per record via the mail) is non-refundable. If the record is located, the search cost does cover the cost of one record.

How much does a copy of a death certificate cost?
Where are copies of death certificates kept?
Massachusetts death certificates are available in the City/Town where the person passed away and in the resident community where the person last resided at the time of death. The original of all records are kept at the Massachusetts Registry of Vital Records and Statistics.
Death certificates DO NOT cross state lines. If the decedent passed away in another state, we would not have the record.
Would Haverhill have a specific death certificate?

How can I obtain a certified copy of a death certificate?
- In Person. The cost is $10 per copy (all forms of payment accepted except American Express, CashApp, and Apple Pay). We are Monday through Friday 8:00 AM to 4:00 PM.
- Electronic Request. The cost is $15 per copy (no extra charge if using ACH, processing fee of approve. $2.50 if using a credit card). We process these requests within the next 1-2 business days.
- By Mail. The cost is $15 per copy, please make a check payable to the "City of Haverhill." We process these requests within the next 2-3 business days. Mail your request along with payment and a self-addressed pre-stamped envelope to:

Amended Death Records
In order for a record to be amended, you must contact the City/Town where the record originated. Death Certificates can only be amended in the City/Town of death.
The cost is $35 for an amendment. Please contact our office for more information regarding evidence required to make the amendment.
